HOW TO ENROLL IN THE SMASA PATIENT PORTAL
To enroll in the SMASA Patient Portal, you must request an enrollment token during your appointment or via our Administrator at firstname.lastname@example.org. You will be required to sign a consent form and provide an e-mail address for use with our SMASA Patient Portal. Once you have an enrollment token, you can click the Enroll Now button bellow to start the enrollment process.
